Psychotherapy, Counselling & Well-being

About me

Welcome! I'm Rhys Yaxley.

With over a decade of experience working in the field of mental health, including more than four years practising as a psychotherapist, I am committed to providing effective therapeutic support.

As a registered psychotherapist with the British Association for Counselling and Psychotherapy (BACP) and Supervisee Psychotherapist with the UK Association for Transactional Analysis, I specialise in offering direct therapy and counselling to young people in care, as well as their carers and care teams.

In addition, I provide therapeutic support to individuals outside the care sector—including children, adolescents, and adults—from a wide variety of cultural backgrounds and life experiences. My practice is open to people of all ages, and I am dedicated to creating an inclusive and supportive space for those facing challenges, whether within or beyond the care system. I often work collaboratively with children’s care teams and parents, especially to support those who have experienced adversity in childhood, ADHD, or trauma.

WHAT TO EXPECT

I will send you some intake forms and questionnaires to gain some background information.

As part of the therapeutic process, I may use validated assessment tools to help us better understand your needs and track progress over time. Two commonly used questionnaires are the CORE 10 and the Adverse Childhood Experiences (ACEs) questionnaire.

The CORE 10 is a brief, standardised measure designed to assess general psychological wellbeing, symptoms, and functioning. It consists of 10 questions and provides a helpful snapshot of your feelings and experiences in recent weeks. Completing it at intervals allows us to monitor changes and tailor therapeutic interventions accordingly. We may continue to complete these at an agreed frequency.

The ACEs questionnaire explores early life experiences that may contribute to emotional and behavioural patterns in adulthood. By identifying past adverse events, we can gain insight into the roots of current difficulties and work together to develop strategies for healing and resilience. Participation in these assessments is always voluntary, and your privacy and comfort are paramount.

CONSULTATION

I offer a free 15-minute video consultation to briefly assess suitability and compatibility to work together.

During your initial consultation, we will take time to discuss your specific circumstances, answer any questions you may have, and consider which type of support best aligns with your goals—whether that’s ongoing individual therapy or a focused parental/carer consultation. This conversation is also an opportunity for you to clarify what you hope to achieve and reflect on any preferences or expectations you bring to the process.

All assessments and sessions are conducted with empathy, discretion, and a deep respect for your lived experience. I recognise that sharing personal information can feel daunting, so we will move at a pace that feels comfortable for you. If you choose to proceed after our consultation, I am committed to fostering a therapeutic environment characterised by acceptance, collaboration, and transparency.

Each step is guided by your readiness and needs, ensuring that therapy is a supportive, empowering experience from the outset.

BEGINNING THERAPY

Beginning therapy can feel like a significant step, and I aim to make the process as welcoming and transparent as possible. After our initial consultation, we will discuss your goals and hopes for therapy, establishing a collaborative plan that feels both achievable and meaningful for you. Our first few sessions are an opportunity to explore your concerns, ask questions, and start building a trusting therapeutic relationship.

Throughout our work together, your comfort and autonomy will remain central. You are always encouraged to share your thoughts on how sessions are progressing and to indicate any preferences regarding the structure or focus of therapy. I adopt a flexible, client-centred approach, blending evidence-based techniques with genuine empathy and respect for your unique journey.

Whether you are seeking support for yourself or looking for guidance on behalf of a young person in your care, my intention is to create a safe, nurturing space where personal growth and resilience can flourish.

Confidentiality in Therapy

Confidentiality is a fundamental aspect of therapy, ensuring that all information shared within sessions remains private and protected. This practice builds trust between therapist and client, allowing for open communication.

Typically, details discussed in therapy are not disclosed to others without your consent, except in specific circumstances such as concerns about serious harm or legal requirements.

To maintain high standards of care, cases may be discussed in clinical supervision. This process is itself confidential and is designed to support good practice and your wellbeing, ensuring that you receive safe and effective therapy
